§ 27-8-12-2 — "Certificate" defined
As used in this chapter, "certificate" means a
document issued to a member of the group covered under a group
insurance policy, which policy has been delivered or issued for delivery
in Indiana, to signify that the individual named in the certificate is
covered under the policy.
