§ 27-13-2-4 — Certificate of authority application; verification; form
Indiana·Title 27 INSURANCE·Art. 13 HEALTH MAINTENANCE ORGANIZATIONS·Ch. 2 Establishment of Health Maintenance Organizations
An application for a certificate of authority to operate a health maintenance organization must be:
(1)verified by an officer or authorized representative of the
applicant; and
(2)submitted on a form prescribed by the commissioner.
