(a)The department shall cooperate with the Indiana department of health and the secretary of state to develop and implement a plan to collect the information described in section 5 of this chapter, IC 16-21-6-3(a)(14) through IC 16-21-6-3(a)(18), and IC 23-0.5-2-13(a)(6).

(b)Before September 1 of each year, the department shall provide the information collected under section 5 of this chapter to the Indiana department of health.
