If the department receives information under section 4 of this chapter from a health care provider or health plan, the department shall keep the following information of the individual who submitted the information confidential:

(1)Name.
(2)Address.
(3)Telephone number.
(4)Electronic mail address.
(5)Personal health information.
(6)Any other information that could identify the individual.
