6. As used in this chapter, "authorization" means a determination by a utilization review entity that:
(1)a health care service:
(A)has been reviewed; and
(B)based on the information provided, satisfies the utilization
review entity's requirements for medical necessity; and
(2)payment will be made for the health care service.
