(a)Except as provided in subsections (f) and
(h), the commissioner shall collect the following filing fees: The commissioner shall deposit fees collected under this subsection into the department of insurance fund established by section 28 of this chapter.
(b)The commissioner shall collect a fee of ten dollars ($10) each time process is served on the commissioner under this title.
(c)The commissioner shall collect the following fees for copying and certifying the copy of any filed document relating to a domestic or foreign corporation:
(d)Each domestic and foreign insurer and each health maintenance organization shall remit annually to the commissioner for deposit into the department of insurance fund established by section 28 of this chapter one thousand dollars ($1,000) as an interna
