3.
(a)An administrator that is licensed
under section 11.1 of this chapter shall, not later than July 1 of each
year unless the commissioner grants an extension of time for good
cause, file a report for the previous calendar year that complies with the
following:
(1)The report must contain financial information reflecting a
positive net worth prepared in accordance with section 11.1(c)(4)
of this chapter.
(2)The report must be in the form and contain matters prescribed
by the commissioner.
(3)The report must be verified by at least two (2) officers of the
administrator.
(4)The report must include the complete names and addresses of
insurers with which the administrator had a written agreement
during the preceding fiscal year.
