§ 25-8-3-16 — Records

Indiana·Title 25 PROFESSIONS AND OCCUPATIONS·Art. 8 BEAUTY CULTURE·Ch. 3 State Board of Cosmetology and Barber Examiners

The agency shall maintain a record of:

(1)the proceedings of the board;

(2)each person licensed under this article, stating the:

(A)name of the person; and

(B)address of the person;

(3)licenses issued under this article, stating the:

(A)number assigned to the license by the agency;

(B)date the license was issued; and

(C)actions taken by the board concerning the license, including any renewal, suspension, and revocation; and

(4)applications for a license under this article that were refused.
