§ 25-8-11-4 — Contents of application
The application described in section 3 of this chapter must state that the applicant:
(1)is at least eighteen (18) years of age;
(2)has successfully completed the eighth grade or received the
equivalent of an eighth grade education;
(3)has graduated from a manicurist program in a beauty culture
school or completed a United States Department of Labor
registered apprenticeship program to practice manicuring;
(4)has received a satisfactory grade (as defined by IC 25-8-4-9)
on an examination for manicurist license applicants prescribed by
the board;
(5)has not committed an act for which the applicant could be
disciplined under IC 25-8-14; and
(6)has paid the fee set forth in IC 25-8-13-9 for the issuance of a
license under this chapter.
