(a)Application for a license issued under this
article shall be made to the board on such form and in such manner as
the board shall prescribe. The applicant shall furnish satisfactory
evidence of qualifications that are required under this article or by the
board. Each applicant shall be notified in writing of the board's
decision concerning the applicant's application within thirty (30) days
after a decision has been reached.
(b)Upon approval by the board of an applicant's application for
examination, a temporary license shall be issued by the board and shall
be in force until the board has notified the applicant of the applicant's
examination results. If an applicant fails to take the next scheduled
examination, the applicant's temporary license is revoked without
