3. An applicant for a certificate as a physical therapist assistant must do the following:
(1)Complete the application process, including payment of fees.
(2)Submit proof of graduation from a professional physical
therapist assistant education program accredited by a national
accreditation agency approved by the board.
(3)Pass an examination approved by the board.
(4)Submit to a national criminal history background check under
IC 25-1-1.1-4.
(5)Meet any other requirements established by rule.
