§ 25-27-1-13 — Physical therapist duties; supervision

(a)A physical therapist is responsible for
managing all aspects of the physical therapy care of each patient. A
physical therapist shall provide the following:
(1)The initial evaluation, determination of a physical therapy
diagnosis, prognosis, and plan of treatment intervention and
documentation of each encounter with each patient.
(2)Periodic reevaluation and documentation of each patient.
(3)The documented discharge of the patient, including the
patient's response to treatment intervention at the time of
discharge.
(b)A physical therapist shall assure the qualifications of all physical
therapist assistants and physical therapy aides under the physical
therapist's direction and supervision.
(c)For each patient on each date of service, a physical therapist
shall:
