Text
(a)A third party logistics provider license
expires at midnight of the renewal date specified by the Indiana
professional licensing agency under IC 25-1-5-4 in each
even-numbered year.
(b)If an application for renewal has not been filed and the required
fee paid before the license expiration date, the third party logistics
provider's license shall lapse and become void.
(c)A third party logistics provider shall provide the third party
logistics provider's latest inspection report, dated not more than two (2)
years from the date of renewal, issued by:
(1)the licensing authority in the state where the third party
logistics provider is located; or
(2)a third party inspection provider.
