(a)The minimum requirements which must be
fulfilled before an applicant will be permitted to take an examination
for a license to practice optometry in Indiana are as follows:
(1)Applicant must be at least eighteen (18) years of age.
(2)Applicant must not have a conviction for:
(A)an act which would constitute a ground for disciplinary
sanction under IC 25-1-9; or
(B)a crime that has a direct bearing on the applicant's ability to
practice competently.
(3)Applicant must be a graduate of a commissioned high school.
(4)Applicant must be a graduate from a college of optometry
under one (1) of the following conditions:
(A)Graduation prior to March 5, 1951, or attendance at or
matriculation in a college of optometry whose requirements for
graduation were equivalent to those required by t
