Text
(a)If a person fails to pay the renewal fee for
two (2) consecutive years, that person's license shall be revoked and
may not be renewed.
(b)If a person whose license has been revoked under subsection (a)
desires to be relicensed, the person:
(1)must submit an application to the board; and
(2)shall be considered as a new applicant.
