1.
(a)The board shall mail an application
for renewal to a licensed dietitian at least ninety (90) days before the
date on which the licensed dietitian's license expires.
(b)The application must be mailed to the licensed dietitian's most
recent address as it appears on the record of the board.
(c)A licensed dietitian filing for renewal of a license must:
(1)satisfactorily complete the renewal application;
(2)return the application to the board; and
(3)submit to the board the required renewal fee;
before expiration of the licensed dietitian's current license.
(d)Upon receipt of the application and fee submitted under
subsection (c), the board shall:
(1)verify the accuracy of the application;
(2)determine whether the continuing education requirement has
been met; and
(3)verify that
