(a)As used in this section, "board" means any of the following boards or commissions:
(1)The medical licensing board of Indiana.
(2)The Indiana state board of nursing.
(3)The state board of dentistry.
(4)The behavioral health and human services licensing board.
(5)The state psychology board.
(6)The Indiana board of pharmacy.
(7)The Indiana board of physical therapy.
(8)The Indiana department of health's consumer services and health care regulation commission.
(b)As used in this section, "license" means:
(1)an unlimited license, certificate, or registration;
(2)a limited or probationary license, certificate, or registration;
(3)a temporary license, certificate, registration, or permit;
(4)an intern permit;
(5)a provisional license; or
(6)a registration; issued by the board r
