§ 25-1-10-1 — "Direct primary care agreement"

As used in this chapter, "direct primary care agreement" means a contract entered into between a primary care provider or an employer of a primary care provider and an individual patient or the patient's legal representative in which the primary care provider or the employer of the primary care provider:
(1)agrees to provide primary care health services to the
individual patient for an agreed upon fee and time;
(2)does not bill a third party that provides coverage to the patient
for the primary care health services; and
(3)charges a periodic fee for the primary care health services.
