§ 20-33-5-4 — Application forms
Indiana·Title 20 EDUCATION·Art. 33 STUDENTS: GENERAL PROVISIONS·Ch. 5 Financial Assistance for Students
The department shall provide each school
corporation with sufficient application forms for assistance under this
chapter. The state board of accounts shall prescribe the forms to be
used.
