Text

(a)Within sixty (60) days after the earlier of:
(1)the expiration of a permit; or
(2)the completion or abandonment of the operation for which a permit was issued; the operator shall file with the director a report of the operation licensed on a form prescribed by the director.
(b)The report must do the following:
(1)Identify the operator and the permit under which operations were conducted.
(2)State the county and township in which the area affected by the operations is located.
(3)Describe the area of land affected by the operation within the time covered by the report with sufficient certainty so that the land may be located and distinguished from other land. A map shall be attached to the report certified by a professional surveyor registered under Indiana law showing the boundar
