After receiving a notice under section 5 of
this chapter, the board of directors of the smaller district shall prepare
and furnish ballots in sufficient number in the following form:
"Shall the _______________ (insert smaller district) Conservancy
District be dissolved and its operations, obligations, and assets be
assumed by the _______________ (insert larger district) Conservancy
District?
[ ] Yes [ ] No"
