§ 14-22-12-5 — Duplicate licenses; electronic affirmation
(a)The department may issue a duplicate
license to replace a lost license issued to an individual under section 1
of this chapter.
(b)A duplicate license under subsection (a) is valid only with the
signature or electronic affirmation of the licensee on the form or device
prescribed by the director.
(c)The department may require a licensee to pay a fee established
by the commission for a duplicate license.
