Text
(a)The division of aging may provide for,
assist in, or authorize the printing of the medical information form and
assembling of a yellow dot folder containing:
(1)the medical information form; and
(2)a yellow dot decal with adhesive backing.
(b)The division of aging shall distribute yellow dot folders to area
agencies on aging and license branches. The division of aging shall
provide information on its website regarding how to obtain a yellow dot
folder from an area agency on aging or license branch.
(c)The division of aging may not charge a fee to participate in the
program.
