§ 12-7-2-127 — "Managed care provider"
"Managed care provider", for purposes of IC 12-14-1 through IC 12-14-8 and IC 12-15 (except IC 12-15-21, IC 12-15-33, and IC 12-15-34), means either of the following:
(1)A physician licensed under IC 25-22.5 who:
(A)is primarily engaged in general practice, family practice,
internal medicine, pediatric medicine, or obstetrics and
gynecology; and
(B)has entered into a provider agreement for the provision of
physician services under IC 12-15-11-4.
(2)A partnership, corporation, or other entity that:
(A)employs or contracts with physicians licensed under IC 25-22.5 who are primarily engaged in general practice, family
practice, internal medicine, pediatric medicine, or obstetrics
and gynecology; and
(B)has entered into a provider agreement for the provision of
