Text
(a)A patient must sign an application if the
patient is medically able to sign.
(b)If a patient is medically unable to sign an application, the
patient's next of kin or a legal representative, if available, may sign the
application.
(c)If no person under subsections (a) and (b) is able to sign the
application to file a timely application, a provider's representative may
sign the application instead of the patient.
