When assistance is granted to a dependent child under IC 12-14-1 through IC 12-14-8, the award made must be entered on a written notice prescribed by the division that designates the following:
(1)The name and residence of the recipient.
(2)The amount of the award.
(3)The date when the assistance is to begin.
(4)Any other fact required by the division.
