The content of the CDL shall include, but is not limited to the following:

(a)A CDL shall be distinctly marked "Commercial Driver's License" or "CDL". It must include, but is not limited to, the following information:
(1)the full legal name and the Illinois domiciliary address (unless it is a non-domiciled CDL) of the person to whom the CDL is issued;
(2)a color photograph of the person;
(3)a physical description of the person including gender, height, weight, color of eyes, and hair color;
(4)date of birth;
(5)a CDL or file number assigned by the Secretary of State;
(6)the person's signature;
(7)the class or type of commercial vehicle or vehicles which the person is authorized to drive together with any endorsements or restrictions;
(8)the name of the issuing state;
