(a)The content of the CLP shall include, but is not limited to, the following:
(1)A CLP shall be distinctly marked "Commercial Learner's Permit" or "CLP" and that it is invalid unless accompanied by the underlying driver's license issued by the State of Illinois;
(2)the full legal name and the Illinois domiciliary address (unless it is a non-domiciled CLP) of the person to whom the CLP is issued;
(3)a physical description of the person including gender, height, weight, color of eyes, and hair color;
(4)date of birth;
(5)the Illinois driver's license number assigned by the Secretary of State;
(6)the person's signature;
(7)an indicator showing that the CLP was issued by the State of Illinois;
(8)the date of issuance and the date of expiration of the CLP;
