(a)A certification confirming a patient's debilitating medical condition shall be written on a form provided by the Department of Public Health and shall include, at a minimum, the following:
(1)the qualifying patient's name, date of birth, home address, and primary telephone number;
(2)the certifying health care professional's name, address, telephone number, email address, and medical, advanced practice registered nurse, or physician assistant license number, and the last 4 digits, only, of his or her active controlled substances license under the Illinois Controlled Substances Act and indication of specialty or primary area of clinical practice, if any;
(3)the qualifying patient's debilitating medical condition;
(4)a statement that the certifying health care professional has confir
