The Department shall establish eligibility standards for the Program, taking into consideration the disability levels and service needs of the target population. The Department shall create application forms which shall be used to determine the eligibility of adults with mental disabilities to participate in the Program. The forms shall be made available by the Department and shall require at least the following items of information which constitute eligibility criteria for participation in the Program:
(a)A statement that the adult with a mental disability resides in the State of Illinois and is over the age of 18 years.
(b)Verification that the adult with a mental disability has one of the following conditions: severe autism, severe mental illness, a severe or profound intellectual dis
