Directories for public health regions; information concerning facility costs and policies.

(a)Each year the Department shall publish a Directory for each public health region listing facilities to be made available to the public and be available at all Department offices. The Department may charge a fee for the Directory. The Directory shall contain, at a minimum, the following information:
(1)The name and address of the facility;
(2)The number and type of licensed beds;
(3)The name of the cooperating hospital, if any;
(4)The name of the administrator;
(5)The facility telephone number; and (6) Membership in a provider association and accreditation by any such organization.
(b)Detailed information concerning basic costs for care and operating policies shall be available to the public
