(a)In order to intercept and divert youth in care from experiencing homelessness, incarceration, unemployment, and other similar outcomes, within 180 days after July 26, 2019 (the effective date of Public Act 101-167), the Department of Children and Family Services, the Department of Human Services, the Department of Healthcare and Family Services, the Illinois State Board of Education, the Department of Juvenile Justice, the Department of Corrections, the Illinois Urban Development Authority, and the Department of Public Health shall enter into an interagency agreement for the purpose of providing preventive services to youth in care and young adults who are aging out of or have recently aged out of the custody or guardianship of the Department of Children and Family Services.

(a) In order to intercept and divert youth in care from experiencing homelessness, incarceration, unemployment, and other similar outcomes, within 180 days after July 26, 2019 (the effective date of Public Act 101-167), the Department of Children and Family Services, the Department of Human Services, the Department of Healthcare and Family Services, the Illinois State Board of Education, the Department of Juvenile Justice, the Department of Corrections, the Illinois Urban Development Authority, and the Department of Public Health shall enter into an interagency agreement for the purpose of providing preventive services to youth in care and young adults who are aging out of or have recently aged out of the custody or guardianship of the Department of Children and Family Services. (b) The intergovernmental agreement shall require the agencies listed in subsection (a) to: (i) establish an interagency liaison to review cases of youth in care and young adults who are at risk of homelessness, incarceration, or other similar outcomes; and (ii) connect such youth in care and young adults to the appropriate supportive services and treatment programs to stabilize them during their transition out of State care. Under the interagency agreement, the agencies listed in subsection (a) shall determine how best to provide the following supportive services to youth in care and young adults who are at risk of homelessness, incarceration, or other similar outcomes: (1) Housing support. (2) Educational support. (3) Employment support. (c) On January 1, 2021, and each January 1 thereafter, the agencies listed in subsection (a) shall submit a report to the General Assembly on the following: (1) The number of youth in care and young adults who were intercepted during the reporting period and the supportive services and treatment programs they were connected with to prevent homelessness, incarnation, or other negative outcomes. (2) The duration of the services the youth in care and young adults received in order to stabilize them during their transition out of State care. (d) Outcomes and data reported annually to the General Assembly. On January 1, 2021 and each January 1 thereafter, the Department of Children and Family Services shall submit a report to the General Assembly on the following: (1) The number of youth in care and young adults who are aging out or have aged out of State care during the reporting period. (2) The length and type of services that were offered to the youth in care and young adults reported under paragraph (1) and the status of those youth in care and young adults.
