§ 42-4011 — NAME OF OWNER ON PERMIT — TRANSFERS RESTRICTED — PERMIT AMENDMENT, FEE

(a)No well may be owned or operated by any person whose name does not appear on the permit or permit application therefor, nor may any well be transferred to any new owner or operator unless an application to amend such permit has been approved by the director.
(b)No changes in or departure from the procedures, locations, data, or persons specified on the face of a permit shall be permitted under this act until an amendment to such permit is approved by the director. The board may specify forms upon which applications for amendments to permits may be filed with the director. Any application to amend a permit shall be accompanied by a filing fee of fifty dollars ($50.00), and the director shall not accept an amendment application which is not accompanied by such filing fee.
