(1)An application for certification shall be made to the department on forms provided by the department and shall contain such information as the department reasonably requires, including, at a minimum, fingerprinting and a background check clearance through the department, a list of individuals living in the home, and payment of an application fee if charged.
(2)Following receipt of a completed application, the department shall conduct an initial certification study to determine the capability of the care provider to operate a certified family home. In the event the department establishes, prior to conducting an initial certification study, that a cause of denial exists pursuant to rules promulgated pursuant to this chapter, the department may choose to deny the application for certific
