Text
1.The application for a license to engage in the business of operating an industrial loan
company shall be in the form as may be prescribed by the superintendent. The application
shall give all of the following information:
a.The name of the corporation.
b.The location where the business is to be conducted, including the street address of the
place of business.
c.The names and addresses of the officers and directors of the corporation.
d.Other relevant information as the superintendent shall require.
2.At the time of making the application, the applicant shall pay to the superintendent

the sum of one hundred dollars to cover the cost of the investigation of the applicant. The
applicant shall also pay to the superintendent the sum of two hundred fifty doll
