1. A lawn crypt may be installed in fewer than ten units if it is installed in an interment space pursuant to a written request to the commissioner signed by the owner or owners of the interment space. 2. The written request shall be filed on a form prescribed by the commissioner and shall contain substantially all of the following information: a. The owner’s name and address. b. The name of the cemetery and the owner of the cemetery. c. The number of lawn crypt units to be installed. d. A description of the interment spaces. e. A statement that the lawn crypt meets the requirements of section 523I.701, including all of the following:
