1.Unless otherwise provided, the question of adopting the proposed alternative form of
government shall be submitted to the electors in substantially the following form:
Should the (charter or amendment) described below be adopted
for (insert name of local government)?
2.The ballot must contain a brief description and summary of the proposed charter or
amendment.
