Text
1.The department shall prepare and upon request supply to police departments,
coroners, sheriffs, and other suitable agencies or individuals, forms for accident reports
required hereunder, which reports shall call for sufficiently detailed information to disclose
with reference to a traffic accident the cause, condition then existing, and the persons and
vehicles involved.
2.Every required accident report shall be made on a form approved by the department if
said form is available.
