The Secretary of State shall thereupon attach to one of the certified copies of the articles of incorporation with order of the judge thereon, which articles of incorporation with the order of the judge thereon constitute the charter of the corporation as granted by the superior court, a certificate in substantially the following form: State of Georgia Office of the Secretary of State Ex Officio Corporation Commissioner This is to certify that __________________ (the name of the corporation) has been duly incorporated under the laws of the State of Georgia, on the __________________ day of __________________, __________________, for a period of __________________ years from said date, in accordance with the certified copy hereto attached, and that a certified copy of the articles of incorp
