With the minor's consent, a
health-care provider licensed, certified, or registered pursuant to title 12 who is
acting within the health-care provider's scope of practice may furnish contraceptive
procedures, supplies, or information to a minor without notification to or the
consent of the minor's parent or parents, legal guardian, or any other person having
custody of or decision-making responsibility for the minor.
