The notice shall contain all of the following:
(a)The name and address of the person to whom the notice is directed.
(b)The date, time, and place of the hearing on the petition.
(c)The name of the minor upon whose behalf the petition has been brought.
(d)Each section and subdivision under which the proceeding has been instituted.
(e)A statement that the minor and his or her parent or guardian or adult relative, as the case may
be, to whom notice is required to be given, are entitled to have an attorney present at the hearing on the petition, and that, if the parent or guardian or the adult relative is indigent and cannot afford an attorney, and the minor or his or her parent or guardian or the adult relative desires to be represented by an attorney, the parent or guardian or adu
