Every license issued under this chapter shall state thereon all the following:
(a)
The name of the licensee.
(b)
The capacity of the license.
(c)
The conditions, if any, subject to which the license is issued.
(d)
The effective date and expiration date of the license.
(e)
If issued to an organization, by separate listing attached to it, the name of each natural person who is qualified to be named thereon pursuant to the provisions of Section 1628.
