Text

Whenever the mother and the other genetic parent or intended parent acknowledges parentage of a child by affidavit, and in the absence of conflicting information on the originally registered certificate of live birth, an application including the affidavits may be filed with the office of the State Registrar upon a form provided for that purpose.
