California·Code HSC Health and Safety Code - HSC·Div. 102. DIVISION 102. VITAL RECORDS AND HEALTH STATISTICS·Part 1. PART 1. VITAL RECORDS·Ch. 4. CHAPTER 4. Delayed Registration of Birth·Art. 2. ARTICLE 2. Application
The application shall be made on the forms prescribed and furnished by the State Registrar and shall contain information and shall be accompanied by affidavits and documentary evidence as required to enable the State Registrar to determine whether the birth did in fact occur at the place and date alleged.
