Text

A. The director shall prescribe the form of application for a certificate of assured water supply, to include, as applicable: 1. The legal description of the land. 2. The plats, plans and maps of the proposed development. 3. Information on the nature and magnitude of the proposed development, including the number of parcels, lots or dwelling units and the annual amount of water which will be required. 4. Evidence of contracts for the delivery of water and information relating to the terms of the contracts. 5. If groundwater is a proposed source of water:

(a)Evidence of ownership of any grandfathered rights pursuant to which groundwater may be withdrawn for the proposed development.
(b)The legal description of the location of any existing wells from which the applicant proposes to
