Text
A.All applications for licenses to practice respiratory care shall be filed with the board.
B.Each application shall:
1.Be accompanied by the prescribed application fee.
2.Be signed by the applicant.
3.Contain a statement under oath of the facts entitling the applicant to take an examination or to receive a license without examination.
4.Contain information the board deems necessary to determine the qualifications of the applicant.
C.If an applicant's application is based on a diploma from a foreign respiratory therapy school or a certificate of license issued by another state, he shall furnish documentary evidence, to the satisfaction of the board, that he has completed courses of study which are at least equivalent to the minimum standards established by the board in its rul
