§ 32-2029 — Fees
Arizona·Title 32 Arizona Revised Statutes·Ch. 19 BOARD OF PHYSICAL THERAPY·Art. 2 Licensure and Examination
The board shall establish and collect fees of not more than:
1.$300 for an application for an original license. This fee is nonrefundable.
2.$300 for a certificate of renewal of a license.
3.$300 for an application for reinstatement of licensure.
4.$50 for each duplicate license.
