A. Each traffic enforcement agency in this state shall use a uniform traffic ticket and complaint form for traffic complaints pursuant to the rules of procedure in traffic cases adopted by the supreme court of Arizona that is issued in books meeting the requirements of this article. B. Each traffic complaint shall contain the following notice: 1. If you fail to appear as directed in this complaint on a criminal charge, a warrant will be issued for your arrest and your license will be suspended. 2. If you fail to appear as directed in this complaint on a civil traffic violation:

(a)A default judgment will be entered against you.
(b)A civil penalty will be imposed.
(c)Your license or nonresident operating privilege will be suspended until the civil penalty is paid. C. The chief adm
