Text
(a)A guardian, in the case of an adult who is an alleged incompetent person, or a parent or guardian, in the case of a minor who is an alleged incompetent person, may petition the court for the sterilization, under this chapter, of an alleged incompetent.
(b)The petition shall state and allege the following:
(1)The name, age, sex, residence, and post office address of the alleged incompetent person;
(2)The name, residence, and post office address of any guardians of the person who is an alleged incompetent person;
(3)The names and addresses, so far as known or can reasonably be ascertained, of the persons most closely related to the alleged incompetent by blood or marriage;
(4)The name and address of any person or institution having the care and custody of the alleged incompetent per
