Text
(a)(1) An application for the issuance or renewal of an ambulance service license or a provisional ambulance service license shall be made on forms provided by the Department of Health and shall be accompanied by any fee as required by law or by rules promulgated by the department.
(2)Each license shall be renewed annually.
(b)Each licensee shall be issued a service license in one (1) of the classifications set forth by the department.
(c)The department shall promulgate rules for the licensure and renewal of an ambulance service license.
